 Learn the skills of conducting effective investigations that take a deep dive to determine what happened and why the event occurred, moving the focus from “who has failed, to what (system) has failed”.
 Understand the investigation process that allows an efficient understanding of the causes and contributing factors that lead to these events.
 Explore the role that human failures contribute to events occurring.
 Learn how to identify any Safe Work System deficiencies, and a process for identifying what lead to the event so that so that effective corrective and preventative actions can be implemented to prevent reoccurrence.
